Every accident is caused by a breakdown in one of four areas: the worker, the tools used, the materials used, or the methods used.
Often there is a breakdown in at least two areas; one being the worker and the other coming from one of the three other areas. The accident's cause usually results from an unsafe act or an unsafe condition
One of the most important aspects of accident investigation is determining the underlying causes of the accident. Often, people stop the investigation too early, forgetting that each main cause may have several underlying causes.
Determining the cause of workplace accidents is extremely important and can ultimately reduce your liability to injured parties, while also determining the root cause and long term solutions.
